Supporting Residents And Families With Care

Facing the end of life is one of the most difficult times for any family. At The Old Vicarage, our focus is on ensuring residents remain comfortable, cared for and surrounded by familiar faces, while families are supported, informed and involved every step of the way. Wherever possible, we support residents to remain in the home they know, with the people they trust around them, rather than facing an unfamiliar transfer at a difficult time.

We work closely with each resident’s GP, district nursing team and specialist palliative care services to make sure care is coordinated, comfort needs are met, and personal wishes are respected and followed.

Providing comfort, dignity and reassurance throughout.

How We Support Residents and Families

Comfort-Focused Care

Our priority is comfort and quality of life. We work with GPs, district nurses and specialist palliative teams to manage symptoms and ensure residents are as comfortable as possible.

Advance Care Planning

We talk openly, and early, with residents and families about wishes and preferences, so that care decisions reflect what matters most to the person, not just what’s clinically possible.

Family Involvement

Families are welcomed and supported to be as involved as they wish to be, with open communication, flexible visiting and a team who make time to listen.

Familiar, Unhurried Care

Residents are cared for by the team who already know them, in the home they know, rather than facing a move to an unfamiliar setting during a difficult time.

End of life care can bring many questions for residents and their families. We’re here to provide clear, compassionate support and help everyone understand what to expect during this important stage of care.

What does end of life care involve?

End of life care focuses on comfort, dignity and quality of life. We provide individual support based around each resident’s needs, wishes and preferences, while also supporting their family.

Can residents remain at The Old Vicarage?

Wherever possible, yes. Our aim is to enable residents to remain in familiar surroundings with people they know and trust, rather than having to move to an unfamiliar setting at a difficult time.

How do you manage pain and other symptoms?

We work closely with GPs, district nurses and specialist palliative care services to help manage symptoms and ensure residents are kept as comfortable as possible.

How are families involved in end of life care?

Families are encouraged to be as involved as they wish. We maintain open communication, make time to listen and work together to ensure the resident’s wishes and needs remain at the centre of their care.

Are personal and cultural wishes respected?

Yes. Individual beliefs, values, traditions and preferences are respected and incorporated into care wherever possible, including any wishes that have been discussed as part of advance care planning.

Can family members visit during end of life care?

We understand how important it is for families to spend time together during this period and aim to support flexible visiting so loved ones can be close to the resident.
